All items below are REQUIRED to process your request: Please provide the following information:

o Your contact information (phone # / email) if additional information is required

o Enclose a self-addressed stamped envelope to mail your AHA Provider card back to you. Use a standard letter #10 envelope. The address you write on this envelope is where your card(s) will be mailed. Postage is REQUIRED, meeting USPS First Class Postage rate.

o Replacement fee of $15.00 for EACH card(e.g. BLS, ACLS, PALS, etc…) + $5.85 Rush fee in the form of cash or check (no credit cards accepted)
